Hospice care brings together a team of people with special skills and everyone works together with the patient, the caregiver, and/or the family to provide the medical, emotional, and spiritual support needed.

Who?

Patients with a life terminal illness where treatment has been discontinued

Where?

Provided at Patient’s home, Nursing Home, Assisted Living Facility, Comfort House, and Hospital (GIP).

Diagnosis?

Cancer, CHF, COPD, Kidney or Liver Failure, Dementia, Parkinson, MS, ALS, Huntington's disease, HIV, CVA/Stroke.

When?

Prognosis of six months or less.

Team?

Physicians, Nursing Team, Social Workers, Spiritual Counselors, Nurse’s Aides, and Volunteers.

Availability?

24 / 7 /365
